Coaches are required to get a Risk Management number from the state. Please go to http://www.wsysa.com/Default.aspx?tabid=93 or call Julie at 253-838-1861.

Coaches please have all members of your team fill out and sign City of Arlington Field Use Waiver form. Forms will be distributed during practice. Forms will need to be returned to club. Please give them to a member of the board or mail them to: P.O. Box 445
